Athletic Schedule Change Request Form - Students must submit this form when adding, droping, or switching sports. Submit the completed form to the Athletics Secretary for processing. The student should continue to attend the sport/class that is on their schedule until they receive confirmation from their counselor that their schedule has been changed.
CVUSD Sport Medical Clearance Form - This form is required when obtaining the student's medical clearance as part of the Athletic Clearance process. The form must be filled out, signed and dated by a doctor, and then uploaded to the student's online athletic clearance at www.homecampus.com.
Student Transportation Form - All student-athletes must ride with their team to/from events for which the school provides transportation. In the case of extenuating circumstance where the student cannot ride with the team, the Student Non-District Transportation Form must be submitted to the AP of Athletics for their approval. The form must be filled out, signed by a parent, signed by the coach, and then submitted to the Athletics Department at least 24 hours in advance of the event. CIF Transfer Form - Students transferring from another High School who want to participate in sports at WHS must complete this form ASAP after enrolling and then submit it to the AP of Athletics, Carnell Henderson (chenderson@conejousd.org). If the student's family moved to a new school attendance area, the Valid Change of Residency Checklist must be printed and submitted along with the CIF Transfer Form as well as the required documents listed on the checklist.
